GranuFlo and Carbon Dioxide Poisoning

In March 2012, the U.S. Food and Drug Administration (FDA) announced a Class I recall – the most serious type of recall – for Granuflo Dry Acid Concentrate and NaturaLyte Liquid Acid Concentrate products. The FDA recall notice linked these dialysis products to several medical problems that are potentially deadly, including increased carbon dioxide levels – called hypercapnia, or carbon dioxide poisoning – in the blood of hemodialysis patients.

The FDA says improperly administered GranuFlo or NaturaLyte can contribute to elevated bicarbonate levels in patients of kidney dialysis (hemodialysis). This can cause metabolic alkalosis, a pH imbalance in which the body has too much of an alkaline substance.

Metabolic alkalosis is a significant risk factor associated with elevated levels of carbon dioxide in the blood, or hypercapnia.

Carbon dioxide, or CO2, is a waste product that we exhale after oxygenated blood courses through the body and returns to the lungs. Hypercapnia is sometimes called “CO2 poisoning.” When oxygen in the blood is replaced by CO2, and oxygen concentrations get too low, we can lose consciousness or even die. Hypercapnia can lead to:

  • Headaches
  • Sweating
  • Malaise (vague feeling of discomfort)
  • Dizziness
  • Dyspnea (breathing difficulty)
  • Increased heart rate
  • High blood pressure
  • Convulsions
  • Coma
  • Asphyxia (suffocation)

According to a June 2012 New York Times report, the manufacturer of the GranuFlo and NaturaLyte dialysis products, Fresenius, had alerted its own physicians and medical directors in November 2011 that GranuFlo “appeared to be contributing to a sharp increase in the risk of patients dying suddenly from cardiac arrest.”

Yet, the manufacturer did not advise the public or the FDA about medical problems associated with GranuFlo or NaturaLyte until the FDA received a copy of the Fresenius memo and inquired about it in March 2012.
